I don't think item 2 would cause this problem. Changing a setting on the source (DVD player, for example) to disable the digital output would do it, as would changing which digital input is used (your item 1). A loose digital cable would cause this, as well. Also, though it sounds like it's unrelated to your problem, some sources like digital cable may not always offer a digital output.
